The Health & Wellness Center has been providing quality healthcare services to our communities since 2005. As a Federally Qualified Health Center (FQHC), we offer Medical, Dental, Behavioral Health, Pharmacy, and Optometry services to those with Private Insurance, Medicare, Oklahoma Medicaid/SoonerCare, as well as the Uninsured/Under-insured, with fees based on household size and income. We currently have five locations in Eastern Oklahoma.

This health center is a Health Center Program grantee under 42 U.S.C. 254b, and a deemed Public Health Service employee under 42 U.S.C. 233(g)-(n).
